- There’s no special form. It’s a written statement that can, but does not have to, be a part of your will.

- The author of the form as well as at least one witness must sign the form.

- Except as specified in a statement written by the deceased, the order of default is: 1) spouse 2) person designated as having power of attorney for the decedent 3) adult children 4) parent 5) adult sibling 6) adult grandchild 7) grandparent
If there is more than one member of a category, i.e. adult children, the majority decision will prevail. - State link
