Any patient can request one from his or her physician.
How do you obtain a DDNR bracelet or necklace
You can contact one of the two approved vendors who have agreed to put the state required information on the items and require the purchaser to submit “Copy 3” of their Virginia Durable DNR Order. Prices start at about $25. The approved vendors are:

Revoking a DDNR
The patient or the person authorized to consent for the patient if that person initiated the DDNR being revoked: By destroying the Durable DNR Order or; Removing the approved Durable DNR bracelet or necklace or; By verbally withdrawing the order.
Photocopy
A photocopy will be honored in the same way as an original.
